What Information is Collected From You?

Client names, phone numbers, email addresses, emergency contact(s), social security numbers, insurance information, doctor information, care plan information, and medical history are gathered once the Service Agreement is completed.

We also are informed of website visitations from our server provider, but this information does not contain any unique identifiers unless one of the forms on the website is filled out.
